Please mail your education/graduation verification form along with the signed release and payment of $15.00 fee payable by company check to 617 N Jackson Avenue, San Jose, CA 95133 attention to The Registrar.
Please make sure that the name your applicant provides is the name used when enrolled at IHS and include the complete date of birth.
We will respond by faxing or emailing you the response if and when we receive your mailed request.
If there is no response that means we have not received your mailed request yet.
